Health Forms
Every camper must have:
A) an up to date medical form from their Dr. (that runs through at least August 18).
Most Dr’s offices will email the forms(s) directly to our office (info@CampCGI.com). If you would like you can print the from below for your Dr. to fill out.
B) Up to date Immunization form from your Dr.
